Abstract
Background
Autoimmunity is a key clinical feature in both post-infectious Myalgic encephalomyelitis / chronic fatigue syndrome (ME/CFS) and Post-Acute Sequelae of COVID (PASC). Passive transfer of immunoglobulins from patients' sera into mice induces some clinical features of PASC. However, the physiological effects of immunoglobulins on cellular alterations remain elusive. In this study, we tested the potential effects of immunoglobulins from ME/CFS patients on endothelial cell dysfunction.
Methods
We have isolated immunoglobulins from 106 individuals, including ME/CFS (n=39), PCS-CFS (n=15), MS (n=20) patients, and healthy controls (n=41). Protein composition of the isolated immune complexes was studied using mass spectrometry. The effect of isolated immune complexes on mitochondria was evaluated using confocal microscopy and a Seahorse XFe96 Extracellular Flux Analyzer, and the impact on inflammatory cytokine secretion was studied using a multiplex bead-based assay.
Results
Here, we demonstrate that IgG isolated from post-infectious ME/CFS patients selectively induces mitochondrial fragmentation in human endothelial cells and alters cellular energetics. This effect is lost upon cleavage of IgG into its Fab and Fc fragments.

The first study to test a digital tool designed to help people with Long COVID manage their energy levels has been developed by a team of researchers.

The paper published in Nature Communications is entitled “A Digital Platform with Activity Tracking for Energy Management Support in Long COVID: A Randomised Controlled Trial”.

In this study, funded by the National Institute for Health and Care Research (NIHR), people with Long COVID tried out a new app called “Pace Me” to help manage their energy levels. The tool combines a wearable activity tracker (Fitbit watch) with an app that sends helpful messages throughout the day, reminding users when they may be doing too much.

Arrival Without a Map

When I first began trying to understand what was happening to my body, everything felt new. Not just new to me, but new in the way that feels destabilizing. As if the ground itself had shifted and no map existed.

Long COVID arrived wrapped in uncertainty. Doctors hesitated. Research lagged. Language failed. Each symptom seemed to require explanation from scratch. I thought I was living at the edge of something unprecedented.

That belief did not last.

Gradually, almost accidentally, I began encountering people who spoke about my present as if it were familiar. They used words I was just learning. Post-exertional malaise (PEM). Pacing. Crashes. Baseline. They described patterns that mirrored my days with unsettling precision. They were not speculating. They were remembering.

This was my introduction to the ME/CFS community. A community that had been here long before I arrived.

Recognition, Not Surprise

The people who live with ME/CFS did not greet Long COVID with surprise. They greeted it with recognition.

Krankenhausleitlinie: Post-Exertionelle Malaise (PEM) - Pflegeanforderungen
KLINISCHE WARNUNG: Wenn sich die Patientin/der Patient in PEM befindet, kann jede zusätzliche Aktivität (Aufsetzen, Gehen, Fragen beantworten) die Symptome verschlimmern, die Erholung zurücksetzen und einen langfristigen Funktionsverlust beschleunigen. Strenger Schutz von Ruhe ist erforderlich.
Übernacht-Pflege
Licht und Lärm: Raum über Nacht dunkel und ruhig halten. Unnötiges Türöffnen vermeiden. Bei notwendigen Kontrollen nur minimales Licht verwenden.
Schlafschutz: Nicht für Routinekontrollen wecken, außer klinisch zwingend erforderlich. Kontrollen bündeln, um Störungen zu minimieren.
Umgebung: Klingel, Wasser und Medikamente vor der Nachtruhe in Reichweite platzieren. Kommunikation: Gespräche und Fragen vermeiden, falls die Patientin/der Patient aufwacht. Leise sprechen, Interaktionen minimal halten.
Eskalation: Bei Schmerzen, Tachykardie oder Instabilität ruhig reagieren, sensorische Reize minimieren und klinisch angemessen eskalieren.
Dokumentation: Nachtdienst dokumentiert Schlafschutzmaßnahmen und PEM-Status zu Schichtbeginn und -ende.
Körperpflege - Duschen und Hygiene
Tägliche Duschaufforderungen sind nicht erforderlich. Zeitpunkt richtet sich nach Belastbarkeit. Grundprinzip: Patientin/Patient signalisiert Bereitschaft selbstständig.
Häufigkeit: Während schwerer PEM evtl. nur einmal wöchentlich oder seltener.

Millions continue to struggle with the lasting effects of Long COVID years after the pandemic began. Researchers at Nova Southeastern University’s Institute for Neuro-Immune Medicine (INIM), led by Dr. Nancy Klimas, are conducting one of the few clinical trials focused on finding a cure for Long COVID. In this study, a monoclonal antibody treatment approved for COVID prevention is being tested as a possible treatment for the complex, chronic Long COVID condition. Over six months, researchers track patient responses, collect biological samples, and evaluate whether this treatment eases chronic symptoms and clears any lingering spike in protein from the initial infection. Funding for the study comes from the Florida Department of Health and the Schmidt Initiative for Long Covid (SILC). Long COVID patients can experience a range of debilitating symptoms that affect the brain, heart, and lungs, including fatigue and cognitive dysfunction, and one hundred patients are participating in this groundbreaking clinical trial to search for a cure.

Hospital Guidance: Post-Exertional Malaise (PEM) - Care Requirements
CLINICAL WARNING: When the patient is in PEM, any extra activity (sitting up, walking, answering questions) can worsen symptoms, reset recovery, and accelerate long-term decline. Strict protection of rest is required.
Overnight Care Guidance
Lighting and Noise: Keep the room dark and silent overnight. Avoid unnecessary door openings. Use minimal light if checks are required.
Sleep Protection: Do not wake for routine observations unless clinically essential. Cluster any checks to reduce disturbance.
Environment: Ensure call bell, water, and medications are within reach before settling for the night.
Communication: Avoid questions or conversation if the patient wakes. Speak quietly and keep interactions minimal.
Escalation: If pain, tachycardia, or instability occurs, respond calmly, minimise sensory exposure, and escalate as clinically indicated.
Documentation: Overnight staff should record sleep protection strategies and PEM episode status at shift start and end.
Personal Care - Showering and Hygiene
Daily shower prompts are not required. Please be guided by the patient as to when capacity allows.
General Principle: Patient will indicate readiness for washing or showering. No need to prompt daily. Frequency: Showering may be limited to once a week or less during severe PEM. Over-exertion can worsen symptoms.
Alternatives: Patient may use wet wipes or gentle seated wash in lieu of shower as tolerated.

PAUL (1/29/2026) — Multiple randomized clinical trials and electronic health record studies now show that metformin, a widely used and well-established medication, significantly reduces the risk of developing Long COVID when taken during or shortly after acute infection with SARS-CoV-2. The findings were published today in Clinical Infectious DiseasesExternal link that opens in the same window.

In the paper, the University of Minnesota Medical School research team provides information about metformin and summarizes several studies in low, standard and high risk populations that show taking metformin during or soon after infection with SARS-CoV-2 prevents the development of Long COVID. Notably, the research summarizes the number of patients needed to receive treatment in each study to prevent one case of Long COVID. On average across the four studies, one case of Long COVID is prevented for every 50 cases of acute SARS-CoV-2 treated with 14 days of metformin.

After infection with the SARS-CoV-2 virus, up to ten percent of those affected in Germany develop Long COVID. The symptoms associated with it, such as fatigue, concentration problems, respiratory issues, or neurological problems, can last for months or years. Furthermore, the clinical picture can differ from person to person. “Long COVID is an extremely complex disease with various manifestations,” says Prof. Yang Li, head of the “Computational Biology for Individualised Medicine” department and director of CiiM. “How and to what extent Long COVID develops is still largely unknown. Figuratively speaking, we are unfortunately only looking at an extremely incomplete puzzle.”

A UCLA-led international research team showed that fragments of digested proteins from the virus behind COVID-19 can attack certain immune cells that have spiky shapes, such as the T cell above.

New research shows that after the body’s defenses kill the virus behind COVID-19, leftover digested chunks of SARS-CoV-2 spike protein can target specific immune cells based on their shape. The revelations could explain why certain populations of cells that detect and fight infection are depleted in patients with severe COVID-19, and shed light on the omicron variant’s milder symptoms.

The study, published in the Proceedings of the National Academy of Sciences, may launch a line of inquiry that informs new strategies for quelling the most serious symptoms of COVID-19. Led by a UCLA team, the scientific collaboration comprises nearly three dozen engineers, microbiologists, immunologists, chemists, physicists, medical researchers and analytical experts.
